The Commonwealth Home Support Program (CHSP) assists older community members to stay strong, independent, and connected while remaining in their own homes for as long as possible. Through this program, Goolburri offers practical support and genuine care that make daily living easier and safer. From help around the home and transport, to social activities and wellbeing services, our support goes far beyond basic assistance. We focus on nurturing connection, dignity, and a deep sense of belonging. Every service is delivered in a culturally safe, respectful, and community-led way—honouring each person’s story, identity, and values. With Goolburri by their side, Elders can continue living with comfort, confidence, and pride in the place they call home.

Domestic Assistance

Domestic Assistance supports Elders to remain independent and comfortable in their own homes. This service provides help with essential daily tasks that an Elder may be unable to complete on their own, ensuring their home environment remains clean, hygienic, and safe.
Clients are required to provide all cleaning products and equipment in good working order for the care worker to use. This helps reduce the risk of injury for both Elders and staff. Care workers will clean and maintain only the areas the Elder uses. In shared living arrangements, this includes the Elder’s bedroom, bathroom, kitchen, and lounge room. All support is delivered respectfully and responsively to meet the Elder’s needs.

Garden Maintenance

The Garden Maintenance service helps ensure Elders’ homes and outdoor areas remain safe, comfortable, and well-maintained. This support includes general yard upkeep and minor home maintenance tailored to the individual needs of each Elder.
All materials or supplies required for maintenance are the responsibility of the Elder, while our care workers provide responsive and personalised assistance to address their specific requirements.

Social Support Group

Our Social Support Group (SSG) helps Elders stay connected, active, and engaged within their community. Through SSG, participants can enjoy a range of group-based activities that promote social inclusion, healthy eating, physical activity, and emotional wellbeing. Transport and light refreshments are provided, and our care workers are available to support individual needs throughout the program.
In addition, our Social Support Individual (SSI) service offers one-on-one assistance either in the home or while accessing community activities. This may include social companionship, support to participate in outings, or short-term respite for carers. SSI support is flexible and can take place in various settings, such as parks, shopping centres, cafés, or other community locations—ensuring each Elder receives personalised and culturally safe support.

Social Support Individual

Our Social Support Individual (SSI) service provides personalised one-on-one support to help Elders remain connected, confident, and active in their daily lives. Support may be offered in the home or while accessing community activities and services. This includes social companionship, assistance to participate in meaningful activities, and short-term respite for carers.
SSI support is flexible and tailored to each Elder’s needs. It can take place in a variety of settings—such as the home, parks, shopping centres, or other community locations—ensuring Elders receive culturally safe and responsive care.

Individual Support Covers:

  • Friendly visits in the home
  • Letter writing and correspondence support
  • Assistance with shopping
  • Assistance with banking
  • Assistance with medication (prompting only, as per CHSP guidelines)
  • Assistance in attending appointments

Allied Health Therapy

Our Allied Health Therapy specialists work as part of a multidisciplinary team to provide personalised, culturally safe care that supports Elders’ health, wellbeing, and independence. Access to this service is available through My Aged Care approval, and home visits can be arranged when needed to ensure convenience and comfort.

Transport

Our Transport service helps Elders stay connected, independent, and active within their community. We provide safe, reliable, and accessible transport for medical appointments, social activities, shopping, and other essential outings. Whether attending health appointments, participating in community programs, or visiting family and friends, our friendly and caring team is here to assist.
Wheelchair access and additional support are available to ensure every Elder can travel comfortably and with confidence. Our care workers respond respectfully and flexibly to each Elder’s individual needs, making every journey as stress-free as possible. With this support, Elders can maintain their independence while continuing to engage in the activities that matter most to them.

Who Can Access This Program

Aboriginal and Torres Strait Islander people aged 50 years and older
Non-Indigenous people aged 65 years and older.

Fees

Our CHSP service fees are kept low and affordable to ensure Elders can access the support they need. Please contact our team for more information or if you have any questions regarding fees.

Referrals

A referral is required to access Support at Home services.
Referrals can be made through the My Aged Care (MAC) Portal.
