At Goolburri, looking after our Elders is at the heart of everything we do. Support at Home (SAH) is a new national aged care program that helps our Elders stay safe, strong, independent, and connected to Country, culture, and community. Goolburri is proud to be a registered Support at Home provider for Categories 1 to 5, enabling us to deliver early to high supports that build independence, wellbeing, and cultural connection. Support at Home replaces the former Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs. It brings services together in a coordinated and culturally respectful way, ensuring our Elders receive the right support, at the right time, in a way that honours their story, strengths, and cultural identity.
The program includes eight ongoing classifications, as well as four transitional classifications for Elders previously on HCP who are waiting to be reassessed. This helps create a smooth and supported transition into the new system for families and community. Services include…

Clinical Supports

Specialised services that help Elders maintain or regain their physical, functional, and cognitive abilities. These services must be delivered or supervised by qualified and accredited health professionals using safe, evidence-based practices.

Who Can Access This Program

Aboriginal and Torres Strait Islander people aged 50 years and over.
Non-Indigenous people aged 65 years and over

Our SAH service fees will be listed on the May Aged Care (MAC) Portal. Please contact our team for more information or if you have any questions regarding fees.

Referrals

A referral is required to access Support at Home services.
Referrals can be made through the My Aged Care (MAC) Portal.
